I haven't been to shikinejima, but have been to kouzushima, but in the
far distant past.

My advice to get ready for a really unpleasant boat ride (if traveling
from tokyo). I'm sure the boats have been upgraded a bit, but here were
our options for the overnight boat trip:

1. sleep out on the deck. get on the boat fast because the kids will all
be grabbing the good real estate (places in the wind shadows)

2. pay an extra 2-man (I think) to get an airline/bus-style seat in the
"first-class salon"

It looks like now they have private rooms for families and so on, but
the trip I went on was memorable.

There seems to be boats from Atami, Yokohama and Tokyo. I'd probably
island hop. I'm assuming you just took the boat from Tokyo and stayed
on board until Kouzushima???

Any advice appreciated...

Yes, we took the "slow boat" from tokyo. Even as poor college students,
the lack of reasonable accomodations for the trip was shocking. We
hopped on with the cheap ticket, and the staff told us we could hunker
down wherever we wanted to. We scouted around, and by the time we did a
circuit of the boat, all the semi-sheltered areas on the deck were
covered with bodies and blankets. So we "upgraded" to the bus seats in
the "cabin" like these, except ours were old grotty plastic.
